Summary
Answering a distress call from a doomed spacecraft plunging toward the planet Karn, the Eighth Doctor tries to save its sole surviving crew member, engineer Cass, only for her to refuse his help outright the moment she learns he is a Time Lord, a race she has come to associate entirely with the horror of the raging Time War. The ship crashes and the Doctor dies alongside her, but he is revived by the Sisterhood of Karn, an ancient order of witch-like mystics whose blood-red elixir can control the regeneration process itself. Their leader, Ohila, offers him a stark choice: regenerate as he always has into another peaceful incarnation, or become something else entirely, a warrior, in order to finally end the war he has spent centuries refusing to fight. Declaring himself “Doctor no more,” the Eighth Doctor drinks the elixir and regenerates into a hardened, unnamed incarnation who will become known only as the War Doctor.

- This mini-episode marked Paul McGann’s surprise return to the role of the Eighth Doctor on screen for the first time since the 1996 TV movie, seventeen years earlier, released online just days ahead of the 50th anniversary special.
- It provides the missing regeneration story between the Eighth and War Doctors, confirming exactly how and why John Hurt’s previously unseen incarnation came to exist.
- The Doctor’s explicit refusal to be called “the Doctor” during this incarnation, choosing instead to fight, recontextualizes the character’s own moral code around the name.
